Recovery Consultants of Atlanta is a private non-profit, 501(c)(3) organization founded in 1999 by a group of concerned, committed, and spiritually centered members of the Metro-Atlanta’s 12-Step and faith-based addiction recovery communities. Over the years, the organization has grown and expanded its services for both clients and patients by offering an array of comprehensive Primary Care Services through its Community Health Center which enhances and supplements all services being provided at RCA. DAY TO DAY AS A PATIENT NAVIGATOR – HOME-ARP SUPPORTIVE SERVICES
As a Full-Time Patient Navigator at Recovery Consultants of Atlanta, your role will encompass a variety of key responsibilities designed to make a tangible impact in the lives of individuals experiencing homelessness. Conduct trauma-informed outreach in shelters, encampments, and transitional housing programs to build rapport and establish trusting relationships with clients. You will assist clients in navigating primary care, behavioral health, and substance use services, accompanying them to appointments and ensuring consistent follow-up care. Your expertise will also be crucial in facilitating connections to social services such as housing navigation and benefits enrollment, while supporting clients through Coordinated Entry assessments as per HUD and local guidelines. Data-driven and detail-oriented, you will accurately maintain client data in HMIS to ensure compliance and document service outcomes. Collaborating with healthcare providers and community organizations, you will advocate for clients, ensuring a streamlined and supportive experience across systems.
