Job Summary:
Responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support following or during a visit to the Emergency Department setting. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the ED Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. The ED Navigator assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes.
Minimum Qualifications:
Bachelor’s degree in nursing and/or any other medically related field such as but not limited to social work, respiratory therapy, physical therapy and medicine. Must have a clinical background with strong problem solving skills. Two-three years of hospital Emergency Department, Care Coordination, and/or Utilization Management experience preferred. Maintain licensure required for specific field.
Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required. Must be able to work with a moderate level of noise.
