Job Description
The Community Resource Navigator, reporting to the Director ofBehavioral Health, will work closely with community partners to identifypotential clients, conduct and assist with needs assessment, collect andrecord required data, provide client case management, connect clientswith services and treatment, and develop a client-centered service plan.
Summary of duties: Identify at-risk individuals, contact them, explainprogram benefits/risks, enroll interested and eligible participants,collect required data, and deliver services. Regularly meet withrepresentatives from across a broad spectrum of partnering communitypartnering agencies such as human services, health care providers, lawenforcement, hospital emergency department, courts, emergency medicalteams, social services, housing agencies etc. to forge and sustaindurable relationships. Assist in coordinating the community’s humanservices and related resources on participant’s behalf. Establishcontact, build and maintain effective relationships with identifiedclients. Assess each client’s history and needs using assignedscreening/assessment tools. Stay connected with clients on a need- andresource-appropriate basis to ensure adherence to service plan. Trackpatient progress through contact and ongoing reassessment, collectingand inputting all required data into the required systems. Incooperation with clients, OCHS clinicians, and communityrepresentatives, develop a treatment plan to engage clients with anappropriate range of service and treatment options. Facilitate clients’navigation and utilization of wrap-around support and healthcareservices. Facilitate and coordinate follow-up case conferences withreferring agencies. Coordinate referrals to community substance abuseservices, and mental health services as necessary. Collect and enter allCommunity Resource Navigator Program data to support assessments,tracking, patient registry, chart reviews, and patient satisfactionsurveys. Actively participate in CRN program, behavioral health,departmental and general staff meetings and/or agency committees asrequired. As assigned, interact with referring resources such asEmergency Department inpatient behavioral health/substance usenavigators from Cape Cod Hospital and other referring organizations inorder to rapidly facilitate coordination and transition to OCHS programssuch as OBAT, Healthy Connections or Care Coordination. Adhere toestablished case management standards of care. Adhere to OCHS policiesand practices. Other duties and projects as assigned.

Harwich Port Community Health Center What we need from you: High Schooldegree required, bachelor’s degree preferred. Experience in engagingclients with mental health, substance use, trauma, and related socialservice challenges Case management, navigation, or community healthworker experience helpful. Reliable transportation and driver’s licenseto travel to different sites Current BLS certification preferredwillingness require Demonstrated passion for and commitment to acommunity health mission Experience in a healthcare setting preferred.
Excellent oral and written communications skills. Proficiency incomputer skills including Epic, Outlook, Microsoft office etc. Excellentjudgment and ability to solve problems in a timely manner. Ability toadhere to strict confidentiality standards. Demonstrated ability to workeffectively both as a team member and independently. Demonstratedability to work in a culturally diverse and inclusive environment
