Recovery Support Navigator (RSN) 4.0 4.0 out of 5 stars 94 Main Street, Hyannis, MA 02601 RSN- Recovery Support Navigator Duffy Health Center operates as a Federally Designated Healthcare for the Homeless Program, which mandates that the majority of the patients served are either currently experiencing or have recently encountered homelessness or other forms of housing instability. The staff members are deeply committed to the mission of the organization, with several individuals possessing pertinent lived experiences that inform their work . Duffy Health Center has received two new federal grants, totaling almost $1 million, that will continue the expansion of our care teams by adding recovery support and community health outreach. Some positions will ensure that each of our two primary integrated care teams in the Duffy clinic has a dedicated Recovery Support Navigator (RSN) , Community Health Worker (CHW), and Registered Nurse (RN) care manager. Qualified candidates must have lived experience with their own substance abuse disorder and must identify as being in recovery. Candidate must demonstrate ability to maintain (2) years of sustained and uninterrupted recovery at time of hire required; 3-5 years of sustained and uninterrupted recovery preferred. Statement of Duties – Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based approach. Position is responsible for providing clinical therapy services to clients, including individual and group treatment at Duffy Health Center’s main site. The Recovery Support Navigator provides non-clinical services that engage, educate and support individuals seeking treatment, and their families, in order to motivate and maintain them in health services. Work includes assessing client needs, developing a Wellness Plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate recovery services, resources, and assistance. The Recovery Support Navigator is considered an essential, non-clinical member of the care team. The RSN is responsible for providing systems navigation within and outside of Duffy Health Center. Patients served include all established and establishing individuals with substance use disorders (SUD) or are on our Medication for Addiction Treatment (MAT) program. The RSN will support in identifying individual goals of recovery, accessing treatment services, and community resources. Education and Licenses Required A candidate for this position must have a High School Diploma or GED as well as at least two (2) years of experience within substance use disorder services, preferably working with the homeless/at risk population; or an equivalent combination of education and experience. This is a full time, 40 hour per week position on site in Hyannis. The schedule would be 4 (four) 10 (ten) hour shifts.
