GREENFIELD CBHC ESP
Case Manager CSO is looking for Case Managers towork in our CBHC (Community Behavioral Health Clinic) Emergency ServicesProgram (ESP.) Our Emergency Service Programs (ESPs) offer clientsbehavioral health assessments, intervention and stabilization (foryouth, families and adults,) 24-hour availability, and are a preferredalternative to hospital emergency departments. We provide dailyassessment and brief counseling, case management, nursing support andpeer support. This position will provide direct case management servicesto CBHC clients. Case management provides follow up support toindividuals evaluated by CBHC program and who remain in the communityafter a Crisis assessment.
You Will:
Identify clients needing casemanagement (CM), focusing support on crisis stabilization, andfacilitating linkages to service supports including CSO, other mentalhealth and SUD providers, medical providers and wellness activities.
Engage Crisis clients either at the time of the initial Crisis encounteror within 24 hours through the client’s preferred method of contact.
Initial engagement is introductory, supportive and educational regardingCase Management services. Provide individualized support based onperson’s unique profile including the individual’s support system,provider recommendations and support needs identified by the individual.
Provide outreach to clients by telephone and in person and through theirsupport network. Complete necessary documentation, releases, andnecessary paperwork. Provide assistance in clarifying transition needs,gaps in services and supports. Assist in locating housing, transport toappointments, and accessing supports for stabilization needs. Assistclients in identifying and using natural supports including family andfriends. Practice a holistic and integrative approach to healthcare thatincludes physical, medical and emotional wellness. This involves anoverview of areas of need and “social determinants of health.” Beresponsive to client requests, flexible and timely in providingservices, creative in finding solutions, and dedicated to a person-firstapproach to care. Collaborate with individual’s providers as neededwithin HIPAA guidelines. Remain engaged with individual after theinitial ESP encounter and until client is stable and engaged inappropriate community supports. Consult with clinical supervisor priorto ending support services. Complete other assigned activities having todo with client care such as hospital searches, insurance verification,telephone triage, and various other Crisis duties.
