Full Job Description Key Responsibilities:
Assist clients with enrollment, re-enrollment, and renewals for CHN and ACA (Affordable Care Act) health programs. Guide individuals through the application process, checking eligibility, completing forms, and updating records. Educate the public about available health coverage options through outreach and community events. Provide culturally and linguistically appropriate services and information. Maintain accurate client data and program reports. Build and maintain partnerships with community groups, healthcare providers, and local organizations. Help clients understand and complete eligibility review forms to stay enrolled in programs. Stay up to date with all training and certifications required by CMS and HHS. Represent CHN positively at all times in the community and at events.
Qualifications:
REQUIRED
Reliable transportation to travel to multiple community events each week.
REQUIRED
Completion of online training and passing the CMS Navigator certification exam. At least one year of experience in case management, social work, healthcare, or a related field preferred. Strong communication and interpersonal skills. Proficient in Microsoft Office (Word, Excel, PowerPoint) and experience with databases. Experience working with uninsured, low-income, or homeless populations is a plus. Bilingual candidates strongly encouraged to apply. Must be organized, detail-oriented, and able to manage time effectively.
