AuthoraCare Collective is currently seeking an Internal Care Navigation Specialist for our Greensboro campus. This is a full-time hybrid position ( Monday – Friday 8:30am – 5:0 0pm ). • Must reside in the Burlington/Greensboro area• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. We are seeking a dedicated and compassionate Internal Care Navigation Specialist to join our team. This role will focus on guiding patients through various internal care programs including counseling, dementia care, home health, home-based primary care, hospice, Medicaid waiver services, and palliative care as well as any future programs. This role will be pivotal in connecting all internal and external communications with stakeholders related to patient care. The Internal Care Navigation Specialist will be responsible for knowing all parameters and protocols for patient eligibility for all programs offered within AuthoraCare Collective. A primary role is identifying timeliness of admission into programs benefiting both patients and payer sources ultimately benefiting AuthoraCare Collective as a serious illness organization. Additionally, this role will have a significant function in the CAP program assisting with administrative functions such as but not limited to processing Medicaid applications. Education/Qualifications:
Bachelor’s degree in social work required or other similar healthcare degree Two years of experience in healthcare case management required Case management accreditation or certification preferred

Valid state-issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation
