Occ Summary Nurse Navigator is responsible for facilitating and assisting patients through the healthcare continuum. This role increases the efficiency and quality of that delivered to the patient. Take a lead role in coordinating the multidisciplinary approach to planning and providing care. Serves as a patient advocate and liaison between the patient/family members and the physicians, surgeons and other healthcare professionals. Delivers supportive care in a personal manner and ensures that patients/family members receive all the needed resources and services required.
