Job Description

Overview The Patient Navigator, RN addresses the needs of the patient from a logistics perspective with a clinical lens, explaining treatment options and providing advice and support to the patient. The patient navigator ensures all disciplines are communicating. Nurse navigators are with the patient and family, guiding them through the care and services within all disciplines/specialties that the patient is engaged. They educate, advocate for, and encourage patients, and their families. The Nurse Navigator connects patients with appropriate care and support personnel, and facilitating access to community resources. Visits patient while in hospital/recovery, follows up after discharge if needed and may follow the patient in support groups for many years to come.
