Short DescriptionBridge the gap between clinician and patient, by providingindividual support and assistance in navigating the health caresystem to provide continuity of care. Working with current RNNavigator disease team to provide non-clinical support regardingscheduling, pre cert process, follow up calls and record retrievalfor timely appointments Call new patients within 24 hours of visitto CCI to check in regarding follow up appointments and needs. Workwith CPN, Navigators, Social Work, Dieticians, Call Center,Providers, Radiology scheduling, chemo scheduling, and phone triageto support patients needs Participate in daily multi disciplinaryrounding on inpatient oncology unit, and identify needs andopportunities for coordination of care, follow up and schedulingAct as a member of the Transition of Care team assisting patientsin transitioning from inpatient to outpatient areas withoutinterruption in services and care Assist in connecting patients toservices for support (ie: social work, financial counseling)Tracking of all patient contacts, follow ups, barriers to care andoutcomes Ensures the delivery of patient care through thecoordination of customer services Participates in interdepartmentaland ancillary activities to ensure quality, cost-effective patientcare. Responsible for all core competencies and Cooper standards ofcar Experience Required Experience working in a healthcare officesetting, or community outreach and education required. Knowledge ofhealth care, inpatient/outpatient settings, Oncology specificpatient population Strong interpersonal skills; demonstratedability to work well with people of various backgrounds,ethnicities, and life experiences. Ability to take direction,follows through with tasks, and work independently. Flexibility,optimism, and a cooperative working style. Education RequirementsHigh School Diploma or Equivalent; clinical training (i.e.

MA, CNA,LPN

) a plus Special Requirements Bilingual Spanish/EnglishPreferred Competent in basic computer skills; knowledge of using anelectronic health records system preferred.
