Full Job Description Overview:

The Patient Navigator acts as a dedicated advocate for patients, helping them navigate the healthcare system and ensuring that appointments are scheduled promptly in line with the guidelines set by the Medical Director. This role involves addressing barriers to care, fostering strong connections with referring healthcare providers, and managing referrals efficiently by securing all necessary medical records and patient information. In addition to supporting individual patients, the Patient Navigator engages with community organizations to promote health education and awareness. They are also responsible for tracking new patient inquiries, compiling monthly data on referral trends, and working closely with the care team to coordinate services. By identifying and removing obstacles to care, the Patient Navigator plays a vital role in ensuring patients receive timely and comprehensive healthcare.

Responsibilities:

What You’ll Do Serve as a patient advocate, guiding individuals through the healthcare system. Ensure timely scheduling of appointments in accordance with protocols established by the Medical Director. Assist patients in overcoming access barriers to care. Build and maintain relationships with referring healthcare providers. Manage all incoming referrals efficiently, including collecting necessary medical records and patient-care documentation. Collaborate with community organizations to promote health education and awareness. Track and document all new patient requests, compiling monthly reports and analyzing referral trends. Work closely with the care team to coordinate services and ensure patients receive the care they need without unnecessary delays.

Qualifications:
What You’ll Bring Required:

Bachelor’s Degree in a relevant field required; may substitute years of experience or navigator training on a year-for-year basis. Minimum of two (2) years of experience in a healthcare setting working directly with patients. Technical clinical experience and/or experience in a specialized practice area (e.g., OB/GYN, Oncology) is a plus. Proven experience in patient advocacy and/or customer service roles. Outpatient care experience preferred. Familiarity with medical terminology is required. Proficient in Microsoft Office and general computer literacy. Excellent communication and interpersonal skills. Strong organizational skills with attention to detail. Ability to work independently and collaboratively within a team. Flexible availability, including the ability to work varied hours. Preferred qualities include being friendly, outgoing, caring, empathetic, dedicated, and resourceful. $61,039-$ 87,183 per year, in accordance with the New Jersey Pay Transparency Law; final compensation will be commensurate with experience and qualifications.
