The Patient Navigator serves as a point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Mount Sinai South Nassau and in the community. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.

Qualifications Responsibilities:

Education Required Must complete in-house orientation program Graduate of an accredited program (Associates Degree required). Must complete annual mandatory in-service education (Hospital/Departmental). Experience And Skills Excellent interpersonal skills Speaks, reads and writes English Impeccable telephone triage skills. Familiar with EMR Computer proficiency (Microsoft Office, minimum typing speed of 40-60 wpm). License/Certifications • Working Conditions Other information: Working Conditions Personal Protective Equipment (PPE) worn Sitting, computer and telephone use for extended periods of time. Frequent, prolonged standing/walking. Lifts, positions and/or transfers patients/equipment. Protective clothing/equipment worn (face shields, gloves, impermeable gown, and particulate respirator).
