Full Job Description Overview:
Westchester Jewish Community Services (WJCS) is seeking a part-time Patient Navigator to support our outpatient mental health clinics. This part-time position pays $25.00 per hour. WJCS is the largest community outpatient mental health provider in Westchester County, dedicated to delivering high-quality, compassionate care to individuals of all ages and backgrounds. Founded in 1943, WJCS provides a vast range of supportive community services each year to 20,000 people of all ages and backgrounds. Responsibilities:
As a valued member of our team, your responsibilities will include, but are not limited to: Working collaboratively with our on-site Licensed Nurse Practitioners as well as the Clinical and Care Coordination Team to provide integrative care. Assisting in the referral process for clients to primary care providers (PCPs). Ensuring that clients’ most recent medical history is accurately entered into the health record. Aiding in scheduling an initial appointment with a PCPs at a convenient time. Helping clients arrange for transportation if needed. Aiding in medical records retrieval and paperwork preparation. Making sure that relevant medical records are shared with our clinic after the annual physical exam takes place.
Requirements:
We are seeking candidates who possess the following qualifications: High School diploma required 2+ years’ administrative staff experience in mental health or medical setting preferred.
