Full Job Description Description:

Care Navigator Companion PRN (as needed) At Bishop Gadsden, we pride ourselves on providing compassionate and comprehensive care to our residents. Located on James Island, SC and we are a not-for-profit, faith-based retirement community, affiliated with the Episcopal Church. Bishop Gadsden is known as the Southeast’s leading life care retirement community. We are located just 10 minutes from downtown Charleston! The purpose of this role is to provide additional support to the Care Navigator team by working with to residents with an emphasis on emotional and social well-being.

A Typical Day:

Promote independence and mobility at all times while providing additional support structure for client’s emotional, psychological and social needs. Develop Person-centered engagement opportunities to meet the unique interests of the resident. Provide transportation at client’s request. Accompany residents to meals at their request. Accompany residents to medical appointments and take notes requested by resident and family. Support the RN Care Navigator with submitting Long Term Care policy claims. Communicate healthcare notes with family members and Care Navigator staff to ensure continuity of care. Observe any change in client condition and report to supervisor immediately. Collaborate with other members of multidisciplinary healthcare team to accomplish plan of care. Document accurately and on a timely basis all information regarding client’s care.

Requirements:
What We Need From A Candidate:

High school diploma or equivalent, preferred. Experience in all aspects of long-term care for the elderly required. Valid South Carolina driver’s license required with no infractions within the past three (3) years. Ability to stand, lift, bend, and stoop for extended periods of time. Ability to read and comprehend labels and written instructions. Ability to communicate with residents, staff, and supervisor. Ability to stand, stoop, bend, reach, squat, climb, reach above, carry, and lift (up to 50 pounds +/-). Ability to operate computer and keyboard.
