Position Title:
Senior Care Manager Work Location:
Remote – South Carolina (MUST reside in SC; may require travel 25% of time)
Assignment Duration:
4 months
Work Schedule:
8:00 AM – 5:00 PM, Monday to
Friday Position Summary:
Transition of Care Case Management for Medicaid members To complete care assessment after hospitalization and provide case management services as needed Working with a great team who collaborates to meet goals
Background & Context:
Support Medicaid Line of Business (LOB) Assist in closing HEDIS care gaps Help increase STAR ratings High level of team interaction via email and chat Hardworking environment with strong team communication
Key Responsibilities:
Conduct member outreaches after hospital discharge within 10 business days Complete care assessments and provide support post-hospitalization Collaborate with team virtually and occasionally through travel Help improve

Qualification & Experience:
Education/Certification Required:
Bachelor’s Degree, Licensed Health Provider Education Preferred:
Master’s Degree Licensure Required:
Licensed Health Provider (RN, LCSW, LMFT, LPC, PsyD, etc., per HEDIS criteria)
Experience Required:
Minimum 2 years as a licensed health provider
Preferred:
Case management experience, psychiatric/mental health background
Software Skills:
MS Office, Word, Excel, Teams Other Requirements:
Must reside in SC; must be willing to travel up to 25% of the time
