Job Description
Overview Qualifications Minimum Qualifications:
One of the following: Bachelor’s degree in pharmacy Pharm D. degree Working knowledge of pharmacy operations, processes, and policies in a hospital and retail setting Demonstrates strong communication, organization and planning skills
Licensure, Registration, Certification:
Current licensure (or licensure eligibility) by the Alabama State Board of Pharmacy (ALBoP)•Permanent licenses will be required within 2 months of entry into the job
Desired Qualifications:
PGY1, PGY2, or Residency completion Experience in Specialty Pharmacy Basic knowledge of 340
B Licensure, Registration, Certification:
Board Certification Preceptor license/eligibility Responsibilities Ensures and provides patient care that optimizes medication therapy and promotes health, wellness and disease prevention through Comprehensive Medication reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy, and improve clinical outcomes.
