This position collaborates with healthcare professionals at Auburn University and East Alabama Health as part of a Transitions of Care program. The primary goal of this program is to reduce hospital readmission rates by providing coordinated, patient-centered care during the transition from hospital to home. Essential Functions Provides medication therapy management consultation and other pharmaceutical care services in a manner that facilitates the achievement of both a high level of patient satisfaction and the attainment of organizational care and service initiatives. Reviews prescribed medications and dosages to ensure that therapy is safe and effective for the patient’s conditions and that all conditions have a corresponding treatment plan if indicated. Monitor and disseminate information concerning program outcomes. Identify, develop, and implement program improvements through a quality improvement initiative. Compile reports provided to grant funding agencies Communicate with other healthcare providers and disciplines to coordinate patient care when transitioning in and out of the hospital.
