JOB SUMMARY
Interprets physician prescriptions and medicationorders. Acts as a drug information resource to patients, medicalstaff, nursing staff and ancillary department personnel. Compoundsand dispenses prescribed medications and other pharmaceuticals forpatient care by performing the related duties.
SUMMARY OF ESSENTIALJOB FUNCTIONS
Reviews prescriptions issued by physicians or otherauthorized prescribers to assure accuracy and determines formulasand ingredients needed. Compounds medications using standardformulas and processes, such as weighing, measuring and mixingingredients. Monitors all posting onto the patient profiles inorder that accuracy, drug allergies, drug interactions, duplicatetherapy, rational drug therapy and incompatibilities may berealized and better patient care received. Performs all aspects ofpatient care in an environment that optimizes patient safety andreduces the likelihood of medical/health care errors. Supports andmaintains a culture of safety and quality. Prepares IV admixturesor directs department personnel engaged in preparing IV admixturesand extemporaneous parenteral products under aseptic conditions.
Answer questions and provides information to physicians, nurses,patients and other healthcare professionals on the appropriate useof medications, drug interactions, side effects, dosage, storage,etc. Endorses policies and procedures of the department and of thehospital. Maintains established procedures concerning qualitycontrol, security of controlled substances and disposal ofhazardous Waste. Enters data into the computer system (Pharmacyprogram), such as patient, prescribed medications, cost andmaintains patient profiles and charge system. Reviews and monitorsdrug therapy that requires dosing adjustments as authorized by thephysician. Provides pharmaceutical care to all ages of patientsfrom newborns to the elderly. Understands that there are dosagevariations between newborn, pediatric, adult and geriatric patientsand knows where to find appropriate information. Assists thePharmacy Director on special assignments. Assists the pharmacyDirector with responsibilities in accordance with theorganization’s mission, policy and any applicable laws.
Demonstrates ability to read, analyze and interpret pharmacyrelated journals. Demonstrates ability to respond to commoninquiries or complaints from customers, regulatory agencies ormembers of the hospital staff. Demonstrates ability to writearticles for publication and ability to effectively presentinformation to management, medical staff, hospital staff and publicgroups. Demonstrates ability to apply mathematical operations tosuch tasks as dosage calculations, formulations of pharmaceuticalpreparations and data analysis. Ensures that pharmaceuticalpreparations are sterile, properly packages, labeled and store andnot outdated. Certifies the skills of the compounding staff.
Maintains air quality check records. Ensures compliance with USP.Medium risk compounding The applicable media-fill test will beperformed and passed annually, after the initial training andmedia-fill test is completed and passed.
BASIC SKILLS
Basiccomputer skills Automated pharmacy systems, typewriters, IV LaminarFlow Hood or the willingness to learn.
EDUCATION AND WORKEXPERIENCE REQUIRED
Doctorate of Pharmacy degree, Bachelor orScience degree or equivalent Graduated from an accredited School ofPharmacy Current pharmacist certification or licensure in the Stateof Texas Education in all age groups ranging from newborn to thegeriatric population obtained either in school education orsubsequent training.
LANGUAGE SKILLS
Able to communicateeffectively in English, both verbally and in writing. Additionallanguages a plus.
PHYSICAL REQUIREMENTS
Must be able to stand, sit,walk for extended periods of time. Must be able to discerncolors.
