Job Title:

Clinical Pharmacist Job Description The Clinical Pharmacy Specialist is dedicated to contributing to the prevention and cure of cancer. This role involves providing safe, evidence-based, cost-effective, and patient-centered medication management as an integral part of the Malignant Hematology interdisciplinary team. The specialist will offer clinical pharmacy support to physicians, nurses, pharmacists, and other staff, both in hospital settings and ambulatory clinics. They will utilize their expertise to recommend, design, implement, monitor, and modify pharmaceutical care plans to optimize patient outcomes. Responsibilities + Provide oversight for safe and appropriate use of medication prescribing, dispensing, administering, and monitoring. + Deliver clinical pharmaceutical care to optimize drug therapy for cancer patients through recommendation, implementation, monitoring, and modification of individualized treatment plans. + Perform medication order review, clarification, and verification, ensuring completeness and appropriateness. + Supervise accurate preparation and distribution of hazardous and non-hazardous drugs per policies and procedures. + Process chemotherapy orders and conduct necessary double checks. + Develop, interpret, and provide medication-related information and education to pharmacists, providers, nurses, and other healthcare personnel. + Coordinate and provide medication therapy education and counseling to patients and caregivers. + Precept pharmacy students, residents, and others in training. + Participate in clinical research and projects. + Lead the development, implementation, and maintenance of medication-related guidelines, policies, and standards. + Manage the medication use process to improve workflow, quality, and safety for patients and staff. + Oversee cost-effective use of medications to ensure appropriate utilization of resources and reduce financial risk. Essential Skills + Doctor of Pharmacy Degree + Board of Pharmacy Specialties (BPS) certification + Extensive clinical experience in disease state specialization + Pharmacy practice experience in medication safety and formulary management + Pathway/guideline and policy development/implementation + Oncology reimbursement and coverage + Clinical research involvement Additional Skills & Qualifications + PGY-2 Residency in Hematology/Oncology + Proficient in medical terminology + Experience with prior authorization processes + Strong background in health care and pharmaceutical care plans Work Environment The work environment is supportive and collaborative, with a strong emphasis on teamwork and patient-centered care.
