Functions 1) Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with pients and providers in assigned mental health care setting. 2) Participes in daily multidisciplinary team rounds with the inpient mental health team and provides drug informion, observions on pient respond to therapy, and approprie recommendions regarding trement alternive and additional interventions to maximize pient outcomes taking into considerion choice of therapy. Safety, efficacy, formulary, and pharmacoeconomics. 3) In cooperion with the mental health team providers, inities alternive drug therapies designed to reduce cost of mediions while still ensuring optimal therapeutic response. 4) May initie, modify, or discontinue medicions with approprie follow-up monitoring under authority given through approved scope of practice. 5) Interviews and/or evalues mental health pients for approprieness of pharmacological therapy (including suicide prevention measures) and will cree and document a pharmaceutical care plan and/or suicide prevention safety plan in progress note. 6) Improve pharmacy services by serving as a direct contact for providers and mental health pients 7) Utilizes evident-based medicine and current guidelines to promote drug prescribing and drug and laborory monitoring to maximize pient outcomes.

Educion:

1) Takes part in self-development activities to improve professional competence. 2) Acts as a drug informion resource for pharmacists and other providers. 3) Develops formal/informal programming for pharmacy staff as required. 4) Provides training to pharmacist, pharmacy residents, pharmacy interns, pharmacy technicians, and pharmacy students. 5) Serves as a preceptor and trains pharmacy resident and students. Evalues resident and students based on performance. 6) Attends and participes in educional presentions including journal clubs, resident presentions and staff development.

Administrive and Supervisory:

1) Responsible for the daily operion stus of various aspects of the pharmacy programs. The incumbent prioritizes work and makes administrive decisions when supervisory pharmacists are not available, or in stances where immedie decisions are required. Pharmacy Service Policy and procedures, VA regulion and ste and federal Pharmacy laws will serve as operional guidance. 2) Serves as a representive on Mental Health committees. 3) Promotes cost effectiveness of formulary and advises on proposed non-formulary criteria for use. Completes non-formulary requests. 4) Represents pharmacy services in a professional manner.

Distribution:

1) Assures the accuracy of drug dispensing and compounding based on pient medicion profile and other pertinent factors in include a review of drug interactions, dosing, contraindicions and formulary stus. For all assignments above the full performance level, the higher the level duties must consist of significant scope, administrive independence, complexity (difficulty) and range of variety as described in this standard the specified grade level and be performed by the incumbent least 25% of the time. Clinical Pharmacy Specialist . The clinical pharmacy specialist (CPS) functions the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for pients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initie, modify or discontinue medicion therapy and as a consultant for intensive medicion therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing. monitoring and documenting, therapeutic plans utilizing the most effective, least toxic and most economical medicion trements; helping achieve positive pient centric outcomes through direct and indirect interactions with pients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laborory and other tests to help determine efficacy and toxicity of medicion therapy.
