• The pharmacist will play a key role indelivering medication therapy management (MTM) services.
• Thisincludes conducting outbound calls to complete comprehensivemedication reviews (CMRs) and providing thorough consultations,particularly around statin use aligned with AHA guidelines.
• Thepharmacist will transcribe and input patient-specific prescription,diagnosis, and recommendation data into a third-party, web-basedplatform.
• This information, drawn from either hard-copy sourcesor internal applications, is used to generate standardizeddocumentation mailed to patients.
• The pharmacist will collaborateas part of the MTM product team and is expected to demonstrateprofessionalism, efficiency, and a patient-focused approach. JobDuties
• Conduct outbound phone calls (approximately 100 calls perday)
• Complete comprehensive medication reviews (CMRs)
• Delivermedication therapy management services
• Accurately and efficientlytranscribe patient-specific data into a third-party, web-basedsystem
• Maintain confidentiality and adhere to HIPAA and privacystandards Estimated Start Date
