Workscollaboratively with preceptors to optimize patient care bypromoting rational, safe, and cost-effective drug therapy bydeveloping and providing clinical services in their area ofexpertise.
Job Responsibilities and Requirements:
PRIMARYRESPONSIBILITIES
- Provides pharmaceutical care in the various areas of clinicalpractice consistent with his/her preceptor.
Duties may include:
review and evaluation of patient drug/medical history, providingdrug therapy recommendations to optimize care, providing druginformation, evaluating medical literature, rounding with otherhealthcare providers, drug utilization reviews, patient counselingand order entry/verification/dispensing.
- Plans, implements, performs and measures the clinical processesthat will improve treatment outcomes while minimizing adverse drugevents and contributes to quality and financial outcomes.
Participates in drug utilization evaluations (DUEs), analyzestrends and evaluates data to be synthesized into reports for actionby committees (if applicable).
- Participates in a culture of learning and sharing that supportshis/her development in the residency program. Implements staffdevelopment on pharmacy processes and participates in the trainingof other pharmacists, pharmacy residents, pharmacy students andhealthcare providers as appropriate for the site.
- Promotes interdisciplinary teamwork including development andimplementation of clinical pathways and protocols that promoteappropriate medication safety and medication utilization using bestpractice, evidence-based recommendations.
- Ensures safety and appropriateness of medication therapythrough order verification, dispensing, home medicationreconciliation, chart review, patient counseling, lab valuemonitoring and dose/therapy adjustment as applicable. Documentsmedication related plans and interventions in electronic medicalrecord.
- Oversees the medication dispensing process to ensure accurateand timely delivery of optimal pharmacotherapy to patients. Thisincludes supervision of pharmacy technicians and/or interns whosupport the role of the pharmacist in the drug delivery system
- Works in a constant state of alertness and safe manner. Performs other duties as assigned

EDUCATION
- Pharmacy degree (Pharm.
D; Bachelor of Pharmacy) from an ACPEaccredited school of pharmacy

PHYSICAL REQUIREMENTS
Frequent lifting/carrying and pushing/pulling objects weighing0-25 lbs. Frequent sitting, standing, walking, reaching and repetitivefoot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near(20 inches or less) and far (20 feet or more) and to identify anddistinguish colors. Frequent use of hearing and speech to share information throughoral communication. Ability to hear alarms, malfunctioningmachinery, etc. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting andgripping. Occasional lifting/carrying and pushing/pulling objectsweighing 25-50 lbs. Rare climbing.
REQUIRED PROFESSIONAL LICENSE AND/ORCERTIFICATIONS

Resuscitation Certification Requirement:
BasicLife Support Healthcare Provider (BLS HCP) required within 90 dayof employment. Other resuscitation certifications may be requireddependent on department.
