Videos Duties The clinical pharmacist is responsible for analyzing real and potential drug-reled medical problems and implementing corrective action to ensure th pients receive optimal drug therapy. The clinical pharmacist also applies standards reling to all aspects of distribution and control of scheduled and non scheduled drugs. The primary function of a clinical pharmacist is to assure the safe and approprie use of medicions, and be an advoce of rional drug therapy through the following: evaluion of the approprieness of drug therapy based on pient specific factors; individualizion of drug therapy; evaluion, dispensing and providing medicions, and drug informion. Duties for this position may include, but not limited to: Provides approprie selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as approprie. Provides pient-specific therapeutic drug monitoring and communices relevant findings and/or recommendions to other health care providers in charge of the pient both orally and in writing. Provides medicion counseling to pients. Completes pient medicion histories, interviews and counseling. Serves as a drug informion resource including pharmacokinetic consultion, including dosing vancomycin and aminoglycosides. Reports ADEs, near misses, and medicion errors in alignment with

Implements P&T Committee policies including therapeutic interchanges and automic substitutions to include approprie pient and provider educion. Manages recalls and medicion shortage situions by substituting alterne dosage strengths and instructions of the same medicion to equal the prescribed dose and schedule. Enters supply orders approprie for pient care per facility policy. Manages various actions, as approprie, for medicion orders. Extends refills until next scheduled appointment under the provider’s name per facility policy. Assumes an active role in staff development of peers and provides educional in-services to nursing, medical staff or to other ancillary services. Reviews all medicion orders for approprieness, drug selection, dosage, route of administrion and the amount, prior to dispensing. Maintains adeque drug stock, inspects drug storage areas, supervises prepacking and the operion of automed dispensing equipment. Supervises the activities of support personnel. Ensures drugs dispensed are completely and correctly labeled and packaged in full compliance with Federal rules and regulions. Provides educion to health care providers regarding medicion use. Licensure:
Full, current and unrestricted license to practice pharmacy in a Ste, Territory, Commonwealth of the United Stes (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registrion if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probionary stus may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.May qualify based on being covered by the Grandfhering Provision as described in the VA Qualificion Standard for this occupion (only applicable to current VHA employees who are in this occupion and meet the criteria).
NOTE:
Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential terminion of such licens e by the Ste for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of th de, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocions or voluntary surrenders of Ste license, registrion or certificion.

Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment the entry level as a Gradue Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B).
