To provide pharmaceutical care that meets or exceeds established standards accomplished through the ability to conceptualize, integrate, and transform accumulated experiences and knowledge into optimal drug therapy outcomes. To provide both clinical and distributive pharmacy services. Performs these functions and other duties as assigned under the general supervision of the Director of the residency program. Education & Experience Pharm D from an accredited school of Pharmacy. Completion of a pre-employment drug screen required. Certifications, Licenses or Registration required: Must be licensed as a Pharmacist in the state of Mississippi or licensed within 90 days after hire date. Possession of a controlled substance license or licensed within 90 days after hire date. Knowledge, Skills & Abilities Responsibilities Compounds, labels and/or dispenses accurately and in a timely manner. Performs computer order entry in an accurate and timely manner. Obtains and enters patient height and weight, allergies and diagnosis into the pharmacy computer system. Follows departmental policy when an order for a non-formulary medication is received. Checks medication carts and code carts accurately as assigned. Properly maintains all legally required records. Recognizes drug supply problems and assists in obtaining and maintaining adequate drug stock. Reviews patient profiles and ensures accuracy of medication administration record. Verifies appropriate and accurate drug distribution tasks performed. Provides concise, applicable, and timely responses to requests for drug information from healthcare providers and patients. Documents patient/family education properly with respect to current medications/devices. Designs, recommends, monitors and evaluates patient specific pharmacotherapy. Assists with formulatory and medication use activities. Provides discharge counseling for patients and documents sessions appropriately. Performs therapeutic drug monitoring, uses pharmacokinetics to evaluate drug regimens and recommends changes in medication regimens to improve patient outcomes. Participates in the health system’s process for assessing, managing, and reporting medication errors, documenting and evaluating adverse drug reactions. Contributes to the training of pharmacy students, other health care professionals and supportive personnel. Delivers effective education and training to patients and the public.
