1st OVN Schedule is set at 830pm•5am Five days a week-rotatingweekends 2nd OVN Schedule fluctuates between 530pm-2am and 830pm-2am. Fivedays a week-rotating weekends. (Fluctuation varies different daysweek to week)

IV Pharmacist Overview:

The IV Pharmacist is responsible for providing consistent andongoing monitoring of all infusion patients. As part of the Infusion Pharmacy team, you will fully assesspatients to determine appropriateness of therapy, high riskmedications, allergies, and other indications or interactions. In this high impact position, you will collaborate with internaldepartments to deliver excellent care coordination with home healthagencies, nursing facilities and physicians. Ensure compliance with all pharmacy laws and regulations, andsterile compounding guidelines. Maintain all required IV documentation and clinical monitoring. Demonstrate thorough knowledge of prescriber/facility notificationprocess for drug interaction, allergy, and other adverse patientsafety situations. Perform accurate dose calculations based on medication, patientcondition, volume and administration rates. Program infusionpumps. Demonstrate thorough competency of medication occurrence reportingprocess. Have a deep understanding of the Polaris Long Term CarePharmacy/Home Infusion Pharmacy Policy and Procedure Manual and internal pharmacy policiesand procedures. IV order processing only when technical support unavailable. Answer drug-related questions as related to IV therapy andmanagement in a timely manner. Communicate with facility consultant pharmacists and clinical staffpharmacists regarding IV patient management. Orient and provide competency assessment for all pharmacists whorotate from oral pharmacy to IV pharmacy and orient and assesscompetency on all IV technicians and infusion pump technicians,when appropriate; this includes making certain that all pharmacistsand technicians rotating into the IV room have completed theapproved Polaris Infusion Training Program. Excellent understanding of the GCPG, formulary initiatives, andPolaris clinical programs. Review consultant customer action request reports and other reportsfor facilities at which IV therapy is provided. Maintain a current knowledge of infusion drugs and therapies andaseptic technique, as well as infusion devices, products, andsupplies. Attend continuing education seminars and annual meetings asprovided to maintain clinical pharmacy skills.

IV Pharmacist Qualifications:

IV Experience preferred Current Pharmacist license active and in good standing in the Stateof practice required Management experience preferred Bachelor of Science in Pharmacy or higher
