Starting annual salary of $120,000•150,000CSI Pharmacy ishiring a Pharmacist to join our dynamic team. CSI Pharmacy is anationwide specialty pharmacy, founded by pharmacists and dedicatedto providing services for patients with chronic and rare illnessesthat require complex care. This role will be based out of our Nash,TX location. The Pharmacist will perform specified distributive andclinical pharmacy activities and related services as they pertainto providing safe and effective home infusion therapies.

ESSENTIALDUTIES AND RESPONSIBILITIES

Adheres to organization policies and procedures. Develops and oversees policies and procedures establishing safepractices for infusion therapy handling in the pharmacy, duringtransport, at the suite, and patient’s home. Coordinates and facilitates new referrals/admissions throughcollaboration with nursing, customer service and distribution. Reviews, receives/verifies new patient orders, and documentsthe same on the respective order form. Assists the physician and other healthcare professionalsinvolved in planning and implementing patient care in the home toensure safe and effective drug therapy. Conducts initial and ongoing assessment of specific patient,drug, and disease information to identify real and potentialdrug-related problems or needs. Assists the physician and nursing staff in setting therapeuticobjectives for drug therapy and establishing parameters formonitoring the safety and efficacy of drug therapy. Monitors the outcomes from drug therapy by collecting,evaluating, and interpreting the results of laboratory data,clinical data other pertinent patient information. Communicates the results of therapeutic drug monitoringactivities to the physician, nursing, and other appropriatepersons. Programs infusion control devices as indicated. Provides consultation and answers inquiries from patients,families, physicians and other healthcare professionals regardingdrugs and their use. Maintains thorough and appropriate documentation in thepatient/client record of all clinical activities and communicationwith patients, physicians, and other healthcare professionals. Participates in the preparation of progress reports for thereferring physician on a regular, periodic basis. Processes and checks medications, supplies and equipment priorto dispensing from the pharmacy. Maintains clinical skills and knowledge of pharmacotherapy byreviewing the literature and attending in-services, seminars, andconferences. Participates in on-call rotation. Performs marketing support functions as requested. Participates in orientation/training programs for new personneland current staff. Participates in departmental QA activities. Attends designated nursing department meetings asscheduled. Maintains pharmacy records in accordance with applicable lawsand regulations. Responsible for handling customer complaints in accordance withCompany policies and advises management promptly. Educate referral sources on all CSI services relating tocustomer needs and benefits. Complete and submit all required reports in a timelymanner. Maintain current files and other records in accordance withCompany instructions and requirements. Collaborates with Senior Management to improve efficiency andproduct knowledge to meet Company goals. Assists management in special assignments, such as market testsand surveys Provides high quality services to the home-care patient and thehome-care referral source.•Attends and participates in conventions, trade shows andin-services as deemed appropriate but management. Consistently represents the company in a professional manner•Maintains effective working relationships and cooperates withCompany personnel. Adheres to the Company’s compliance requirements as stated inthe policy and procedure manual and all other relatedpolicies. Perform other duties and responsibilities asassigned.

QUALIFICATION REQUIREMENTS

EDUCATION AND/OR EXPERIENCE

Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Masterof Science degree in Pharmacy desirable) from an accredited collegeof pharmacy. Currently licensed as a registered pharmacist in state ofemployment A minimum of 2 years in specialty pharmacy OR ICU/critical careexperience•plasma-derived therapy experience is highlypreferred. Must be capable of assuming responsibility for evaluating andmonitoring drug therapy relative to the total needs/problems of thehome care patient in collaboration with physicians, nurses, andother healthcare professionals.
