Job Description:
Responsible for the accountability of dispensing prescription medications to patients as prescribed by physicians, maintenance of accurate records and providing expertise in the safe use of prescription medications
- Fill prescriptions, verify instructions from physicians on the proper amounts of medication to give to patients
- Check for negative interactions with current medications patient takes or any medical conditions the patient has
- Review written prescriptions for safety, efficacy, appropriateness, and accuracy; establish or maintain patient drug profiles to avoid adverse reactions.
- Complete insurance forms and work with insurance companies to ensure that patients get the medicines they need
- Advise patients about general health topics, such as diet, exercise, managing stress, and on other issues, such as what equipment or supplies would be best to treat a health problem o Monitoring customers’ drug therapies, advising interventions, and educating patients, caregivers, staff and other professionals regarding appropriate medication use and any potential side effects
- Design, recommend, monitor and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients
- When appropriate, make suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters for a home care patient.
- Collect and organize patient-specific information needed to prevent, detect, and resolve medication-related problems and to make appropriate medication therapy recommendations.
- Identify appropriate monitoring parameters for each patient.
- Use effective patient education techniques to provide counseling to home care patients and/or their caregivers o Responsible for reviewing prescribed medications performing clinical and drug utilization reviews and clinical/economic evaluation of therapies and writing criteria sets for medications that have a prior authorization attached to them.
- Perform total patient medication assessment as requested for new enrollees identifying interventions to address polypharmacy, drug-drug, drug-disease interactions, high risk medication use and non-adherence
- Identify opportunities for cost saving interventions utilizing high cost medication and medical cost lists
- Review and provide clinical decisions for provider submitted prior authorizations for medically administered medications as well as pharmacy related appeals and grievances.
o Provide pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. Work with the team to ensure compliance with Federal, State and local laws and regulations as well as proper storage and documentation of pharmaceuticals.
- Comply with state/federal laws and USP standards, including Drug Enforcement Administration (DEA) controlled substance regulations set forth by the state and DEA. Comply with policies regarding the perpetual inventory systems for schedule II narcotics, controlled substances and other medications subject to diversion.
- Interpret medical orders (verbal and written) and transcribe to computerized patient medication profiles accurately.
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Delegate tasks to pharmacy technicians and other support staff, as permitted by regulation and policy, while providing professional oversight o Providing drug information and assist physicians in providing appropriate drug therapy for pediatric, adult, geriatric patients.
- Provide clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
- Provide accurate, adequate, and timely drug information to the professional staff.
- Provide expert information to physicians, patients, nurses, and others about drugs and other pharmaceuticals with respect to such factors as the appropriateness of drugs in relation to the patient’s condition and drug history; the choice of drugs including dosage form, quantity, methods of administration, and alternative therapy for drugs having undesirable side effects; the pharmacological characteristics of drugs including their effects on the human system and the condition being treated and their chemical interaction with other substances in the patient’s system; and their physical characteristics such as compatibility with other drugs, forms in which they are or can be manufactured, and expected shelf life.
o Additionally, responsible for maintaining standards.
- Ensure compliance with all federal, state, and local regulations (i.e. HIPAA)
- Ensure compliance with all company policies and procedures
- Verify that prescription is from a licensed prescriber
- Accurately and efficiently prepare, compound and dispense prescriptions for customers according to legal requirements (includes product selection, counting and measuring drugs, capping and uncapping vials and bottles and labeling of prescriptions)
- Advise customers regarding instructions for medications prescribed by physicians and recommend use of OTC (over the counter) medications
- Maintain proper storage and security conditions for all drugs
- Determine the most efficient and cost-effective medications and alternatives, review inventory, place orders, and verify receipt of medications. o Help create an optimal environment to facilitate non-dispensing activities (i.e. immunizations, Medication Therapy Management (MTM), specialty medications care management, population health initiatives conducting health and wellness screenings, etc.).
- Demonstrate the ability to process Outpatient/Retail prescriptions in a safe, accurate, efficient, and regulatory compliant manner while adhering to designed Outpatient/Retail pharmacy workflows (i.e. data entry, DUR evaluation and resolution, PBM claim adjudication, etc.)
- Efficiently conduct MTM and immunization opportunities as allowed by federal and state government
Job Requirements:
Bachelor’s Degree is required in Pharmacy or Pharmaceutical Science and an active State License.
