Job Description
A leading provider of home infusion and specialty pharmacy services is seeking an experienced and detail-oriented Infusion Pharmacist to join a team in Tennessee. This is a vital role supporting patients with complex therapies in a home-based care model. Must hold an active/clear pharmacist license in the State of Tennessee.

Full-Time, Contract or Travel Key Responsibilities:
Prepare, compound, and dispense sterile IV medications according to USP and standards Review and evaluate physician orders for clinical appropriateness and accuracy Educate patients and caregivers on medication administration, side effects, and care protocols Collaborate with nurses, physicians, and care coordinators to ensure high-quality outcomes Maintain full compliance with state and federal regulations and accreditation standards Participate in rotating on-call coverage for after-hours clinical support Manage medication inventory, expiration tracking, and storage standards
Qualifications:
PharmD or Bachelor’s degree in Pharmacy from an accredited institution Current, active pharmacist license in the state of Tennessee 2+ years of experience in home infusion, hospital, or sterile compounding pharmacy highly preferred Solid understanding of infusion therapies and aseptic techniques Strong communication, documentation, and patient-care skills Familiarity with infusion pharmacy software systems is a plus
