Pharmacy :
The incumbent is responsible for general clinical pharmacy support of the Gerirics PACT clinic, including directing formulary use, cross-coverage of pharmacy managed clinics, and assuring compliance with nional performance measures, while serving as Pharmacist provider assisting Physicians, Nurse practitioners, and Physician assistants in active Pharmacotherapy management of individual pients. Duties include and are not limited to: Develops and maintains an in-depth clinical knowledge of common geriric care chronic disease stes and antithrombotic/anticoagulion management including pharmacotherapy, laborory, monitoring, and outcomes management. Assists in the global and daily administrion of the medical center Geri-PACT clinic services. Assists in prescription/drug regimen review and pient educion of assigned Geri-PACT pients. Develops professional relionships with non-pharmacy clinical personnel, and projects the highest level of pharmacy practice and competence. Monitors and manages pient medicion therapy utilizing advanced skills in physical assessment, pharmaceutics, pharmacokinetics, pharmacodynamics, and interpretion of laborory and other diagnostic parameters. Provides direction to Geri-PACT providers on nional formulary management issues including compliance with nional pharmacy performance measures. Reviews Geri-PACT non-formulary requests against nional criteria for use for approval/disapproval recommendions. Reviews and evalues prescribed drug therapy for approprieness, effectiveness, cost-benefit, adverse drug reactions, allergies, drug-drug and drug-disease interactions, and communices and documents recommendions to providers and pharmacists as necessary. Appropriely renders pharmaceutical care to the Geri-PACT and anticoagulion pients and documents care in the medical record. In coordinion with the multidisciplinary team, collabores in determining an individual trement plan for each Veteran in Geri-PACT clinic with periodic assessment, and approprieness of pients for any self-medicion program with emphasis on meeting the individualized Veteran’s medicion management needs, as well as discharge planning to promote veteran educion and skill development designed for improved self-care. Identifies, interprets, and resolves prescribing and dispensing issue including pharmacotherapeutic and formulary issues. Documents interventions as necessary. Designs and performs relevant drug use evaluions under direction of the Clinical Pharmacy Coordinor. Participes in research studies in ambulory care areas as assigned.

Licensure:

English Language Proficiency:

GS-13 Clinical Pharmacy Specialist Experience . Must have 1 year of experience equivalent to the next lower grade level th includes: (Must be documented on your resume.) Knowledge of laws, regulions, and accredition standards reled to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. The clinical pharmacy specialist (CPS) functions the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for pients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initie, modify or discontinue medicion therapy and as a consultant for intensive medicion therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medicion trements; helping achieve positive pient centric outcomes through direct and indirect interactions with pients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laborory and other tests to help determine efficacy and toxicity of medicion therapy.
