POSITION SUMMARY

Perform functions of a Certified Pharmacy Technician interacting with partnered members and care givers via virtual options. Collaborate closely with members, pharmacists, and physicians to increase the members understanding of current medications and medication adherence, while working within the limits of established policy. Perform outreach to patients regarding medication management and chronic illness management.

ESSENTIAL FUNCTIONS

Communicate the details of C³HealthcareRx program features, objectives, and benefits to members and/or caregivers – telephonically. Complete patient health questionnaires and an accurate medication reconciliation to include prescription and over the counter medications during telephonic-home visits. Develop rapport with members and/or caregivers to gain confidence and trust. Collaborate with the consultant pharmacist as well as member’s physicians to increase the members understanding of current medications and compliance. Communicate the details of the company programs to patients and provide excellent customer service. Update patient records in electronic medical record system. Accountable for following department guidelines, company policies, and HIPAA privacy rules. Provide excellent customer service to members, caregivers and partners. May be required to provide support in closed door pharmacy performing inventory, shipping, dispensing, and patient management functions. Perform other related tasks as assigned.

EXPERIENCE / EDUCATION REQUIREMENTS

High School diploma or equivalent. Current and in good standing Pharmacy Technician Certification in the state of NC. Two (2) years of pharmacy tech experience with National Certification (PTCB or equivalent) strongly preferred. Experience in communicating to patients telephonically with issues concerning medication adherence and chronic care.
