Videos Duties The Pharmacy Technician (Telephone/Community Care) Duties include: Receives faxed, mailed, electronically prescribed, or delivered prescriptions written by non-VA providers. Reviews Community Care authorizion of Non-VA/Community Care. Communices with pients concerning prescriptions received from unauthorized provider s and handles prescriptions appropriely, whether forwarding to primary care provider or returning pient. Reviews authorized prescriptions for approprieness using pharmacy technician knowledge, as well as knowledge concerning the nions drug formulary and local VA formulary requirements. Inities non-formulary/prior authorizion process with Non-VA/Community provider and records needed documentions. Routinely reviews computer medicion profiles for therapeutic duplicion, prescription non-compliance and allergies/drug reactions. Transfers identified pients to a pharmacist supervisor, for further evaluion, if needed. Communices with Non-VA/Community Care physicians and nurses to request clarificion of prescriptions or medicion changes due to VA formulary and forwards all changes and clarificions to a licensed pharmacist to receive frim the Non-VA/Community Care representive. Routinely reviews computer medicion profiles for therapeutic duplicion, prescription non-compliance and allergies/drug reactions. Transfers identified pients to a pharmacist or supervisor, for further evaluion, if needed. Processes and submits pient requests for new prescription orders and renewals (including controlled substances) into the computer accurely, thoroughly and in a timely manner. Uses approved Pharmacy Service protocols and standards of practice when approprie to sisfy pient medicion requests. Consistently documents pient contact and call resolution in the electronic healthcare record as applicable, to include but is not limited to wh was requested, wh action was taken and any other informion which may be pertinent during future research, Assists in the documention of medicion variances. Seeks assistance from designed pharmacist or supervisor when responding to unusual questions or when reasonable tempts to sisfy a caller have been unsuccessful. Interviews pients, family members and/or caregivers, to develop an accure list of medicions, drug strengths, dosages, routes, and instructions the pient has been taking. Compares the compiled list with the active medicion profile in the computerized medical record of the pient. Updes and clarifies allergy informion in the pient’s records in an electronic health record (EHR). Participes in quality assurance, process improvement activities, and performance monitors. Compiles, cegorizes, records and analyzes workload da for various reports in order to establish a comprehensive record of work performed. Contacts and collabores with other healthcare providers in the facility as deemed approprie. Maintain competence by participing in staff development programs.
