Responsibilities:
The Physical Therapist is responsible fordirect patient services, including evaluation and treatment,participation in in-service programs, recordkeeping, anddocumentation and provides consultation to staff and family whennecessary

Evaluation:
Performs pre-entry evaluation, initialevaluation, and re-evaluations to assess childs development level,and identify handicapping delays and disorders. Establishes longand short-term goals for each child

Treatment:
Plans andadministers individual physical potential and development. Modifiestherapy program as needed

Written Reports:
Provide writtenreports of all evaluation, re-evaluations, progress reports, dailyfile notes, and annual reviews as scheduled.
Qualifications
- Mustpossess a minimum of a B.S. in Physical Therapy from an accreditedcollege or university
- Must be licensed and currently registeredin New York State as a Physical Therapist or possess a limitedpermit to work under a Licensed Therapist
- Excellent communicationand interpersonal skills for effective interaction
- Ability to workindependently and as part of a multidisciplinary team
- Strongorganizational skills for case management and documentation.
