ResponsibilitiesResponsible for helping patients maintain,restore and improve movement, activity and function.
Providesinterventions, conducts reexaminations (every 30 days, at minimum),modifies interventions as necessary, and performs dischargeplanning.
Other duties as assigned.
QualificationsCurrent license topractice physical therapy specific to the state you’re assigned towork in.
Graduate from an accredited physical therapy educationalprogram as set by the Commission on Accreditation of PhysicalTherapy Education (CAPTE).One year related physical therapyexperience for PTs entering home health care to ensure thatknowledge and skills are sufficient to safely provide physicaltherapy services to patients.
If less than one year of experience,approval from regional clinical leaders is required as well asappropriate mentoring and/or participating in a residencyprogram.
Current CPR certification.
