PhysicalTherapist What You’ll Do:

Provides physical therapy services topatients according to a written physician’s plan of care and asdefined in the state Physical Therapy Practice Act.

Advise, consultand teach appropriate treatment procedures and supportiveactivities to clients, family members and other agency staffAssistthe provider in evaluating level of functionParticipate in thedevelopment of the plan of carePrepare clinical and progressnotesTranslate all exercise into functional activities oractivities of daily livingDevelop needed plans for modifyingequipment, appliances and the physical surrounding in clientshomeConfer regularly with attending provider regarding client’sprogress and report the physical and emotional conditions andreactions to treatment and interpret social and physical factors inthe environment that affect careInterpret to the client and familythe implication of the treatment consistent with the actions andwishes of the providerProvide families with information, supportand encouragement to help motivate them in their progressPreparesand submits clinical and progress summaries based on the attainmentof goals.

Supervises physical therapy assistants according toorganization policy and state regulations.

Maintains accurateclinical records, keeping track of goals and progress ofclients’

Qualifications:

Physical TherapistGraduate from a School ofPhysical Therapy approved by the American Physical TherapyAssociation or Allied Health Education and Accreditation of theAmerican Medical Association and the Physical TherapyAssociationCurrent licensure as a Physical Therapist in the stateof practiceAt least two (2) years’ experience in an acute hospitalor equivalent experience (preferred)Must possess current CPR andFirst Aid CertificationMust have reliable transportation, currentdriver’s license and appropriate automobile insurance
