Position Overview Aveanna Healthcare has grown tremendouslyduring the past year through a series of exciting acquisitions andwe are currently looking for a Physical Therapist (PT) to join ourdynamic home health care team. In this position, you make itpossible for patients to recuperate from illness or injury in thecomfort of their own home. Up to $75 per routine visit and $93.75per evaluation visit. Why Join our Team? Essential Job Functions:
- Perform initial andongoing assessments to evaluate mobility, coordination, strengthand current level of function.
- Responsible for administration,evaluation, and interpretation of tests and measurements todetermine functional goals and outcomes.
- Provide a writtenevaluation and plan of care defining physical therapy needs, goals,progress and expected outcomes.
- Delivery of physical therapyservices to comply with prescribed physician orders.
- Maintainsplan of care by performing timely documentation of clientperformance, modifying goals and treatment as needed, and determinedischarge care plans as appropriate.
- Responsible for dischargeplanning, including collaborating and educating the patient andhis/her caretakers on home exercise plan to follow discharge.
- Participates in coordination of services with health care team,physicians, parent/guardians, nurses, etc.
- Documents according topolicy and procedure and requires minimal supervision orinstruction.
- Documents effectively resulting in no technical orclinical denials subsequent to review of documentation by payers
- Maintain effective communication between staff, healthcare teammembers and family. Serve as a resource consultant for clinicalstaff.
- Supervises other personnel (PTA, HHA) as applicable.
- Completes OASIS assessments where allowed by state professionalpractice and regulation.
- Follow policies and procedures andconforms to all applicable patient care standards, licensureregulations and state laws.
- Maintain effective communicationbetween staff, healthcare team members and family. Serve as aresource consultant for clinical staff.
- Supervises otherpersonnel (OTA, HHA) as applicable.
- Completes OASIS assessmentswhere allowed by state professional practice and regulation.
Requirements:
- Degree in Physical Therapy from an accrediteduniversity/program.
- Current, unrestricted state license as aPhysical Therapist in the state of practice
- Demonstratedproficiency in clinical assessments, documentation and compliance
