StaffInterprets physician referrals, as appropriate, and completesmusculoskeletal, neuromuscular, cardiopulmonary, and/orintegumentary evaluations utilizing appropriate methods.

Establishes a plan of care and implements individual treatmentbased on evaluation results and functional goals and by utilizingappropriate techniques. Monitors and evaluates outcomes andappropriately adjusts the individualized treatment plan based onthe patient’s response to treatment and provides status updates tophysicians and other teammates. Identifies factors affectingpatient’s physical and psychosocial comfort and attends to theseneeds. Plans for and completes patient discharge includingproviding written home programs, patient/family teaching, referralfor community services, transition planning within the continuum ofcare and ordering equipment as indicated/appropriate. Communicatesand collaborates with appropriate stakeholders to assist inachieving optimum care for the patient. Completes all requireddocumentation following department policy and accreditationstandards with appropriate co

  • signatures, including patientevaluations, progress notes, team conference reports, and dischargesummaries as appropriate.

Record patient care and progress of eachpatient assigned. Completes daily statistics, billing, andscheduling. Applies insurance guidelines regarding authorizationand number of visits. Responsible for maintaining site/departmentestablished productivity standards. Responsible for understandingand adhering to all billing and documentation policies, procedures,and timelines. Responsible for cleaning up after patient treatment,putting away equipment, and keeping area neat and orderly. Performsvarious manual techniques, patient positioning, transferring andtransporting duties, which require lifting and pushing/pulling,while utilizing proper technique. Demonstrates skill in use ofmodalities, exercise equipment and other devices that are availablein area worked per department orientation checklist. Demonstratesknowledge of indications and contraindications of modalities andexercise. Promotes patient safety by participating in safetyinitiatives as requested and appropriately follows all safety andinfection control measures pertaining to the general workingenvironment. Participates in performance improvement activitiesdesigned to identify and resolve problems affecting patient careoutcomes and to improve quality of care. Manages assigned caseload,informing site leadership of coverage needs and ability to cover;assists with coverage, based on department needs and regional needsPursues professional growth through participation in continuingeducation courses, department in-services, and reading professionalliterature. Orients new staff to clinical and department policiesas requested. Participates in the evaluation process of co-workersby providing verbal or written feedback as requested. May supervisephysical therapy assistants following all applicable state andlocal compliance requirements. Includes delegating patients toassistive personnel while providing direction, recommendationsregarding treatment and ongoing supervision while maintainingresponsibility for patient plan of care and goals. May supervisephysical therapy extenders (rehab aides/techs) in administeringtreatments to patients and will teach them appropriate techniquesto perform with specific patients. May supervise physical therapystudents after completing one year of clinical experience. MinimumJob Requirements Education Doctorate degree in Physical Therapyfrom an accredited school or educational requirements commensuratewith the date of graduation. Maintains continuing educationrequirements as mandated by applicable state professionalstandards.

Certification / Registration

/ License Licensed PhysicalTherapist in the State upon which you work. Basic Life Support(BLS) for Healthcare Providers (obtained within 90 days unlessdepartment leader has determined is not required) by American HeartAssociation (AHA) Work Experience No experience required Knowledge/ Skills / Abilities Intermediate computer skills Excellentcommunication, interpersonal, and organization skills Demonstratedorganization, decision-making, and problem-solving skillsSuccessfully completes all competency assessments/requirementspertaining to provision of patient care and consistent with sitestandards Team members will maintain annual educationalrequirements for hospital-based certifications (e.g. DNV, CARF orothers). Must be able to demonstrate knowledge and skills necessaryto provide care appropriate to the age of the patients served. Mustdemonstrate knowledge of the principles of growth and developmentover the life span and possess the ability to assess datareflective of the patient’s status and interpret the appropriateinformation needed to identify each patient’s requirements relativeto his/her age-specific needs.
