As a Home Health Physical Therapist , youwill: Plan and administer prescribed skilled physical therapytreatment and training for patients suffering from variousinjuries, illnesses and functional disabilities to attain highestlevel of physical function. Test/screen the patient’s physicalstrengths to assist the physician in evaluating the patient’s levelof function and records findings to develop or pursue treatmentprograms and establish measurable training objectives.
Develop/implement a conditioning/rehabilitation program consistentwith physician’s Plan of Treatment and the overall goals of thepatient/rehab team. Adjust treatment as needed to achieve maximumresults. Confer with physician and clinical team members to obtainadditional patient information and assist in developing,implementing and revising the therapy treatment program and Plan ofTreatment. Provide Physical Therapy Assistants and Home Health Aidestaff with written instructions/care plan that reflects currentplan of care as related to therapy, supervise/evaluate staffs’performance. Monitor the appropriate completion of documentation byphysical therapy assistants and home health aides/personal careworkers as part of the supervisory/leadership responsibility.
Accurately, promptly and thoroughly document patients’ careobservations, interventions and evaluations. Assure that interim(verbal) orders received from the physician and physical therapistare promptly and accurately documented, submitted for physiciansignature and implemented Report patient’s progress to thepatient’s physician, Clinical Manager, staff, patient and family.
Submit evaluation, treatment plans, progress reports and dischargesummary to the supervisor and care management staff. Use yourskills to make an impact
Required Experience/Skills:
Degree from anaccredited Physical Therapy Program (approved by the APTA) Minimumof one year physical therapy experience preferred Current andunrestricted Physical Therapy license Current CPR certificationStrong organizational and communication skills A valid driver’slicense, auto insurance, and reliable transportation are required.

- $117,500 per year base pay Scheduled Weekly Hours 1 Pay Range Thecompensation range below reflects a good faith estimate of startingbase pay for full time (40 hours per week) employment at the timeof posting.
The pay range may be higher or lower based ongeographic location and individual pay will vary based ondemonstrated job related skills, knowledge, experience, education,certifications, etc. $85,400
