Education:
Bachelor of Science Degree in physical therapy from anAPTA approved program.
Experience:
One year of acute careexperience. Additional experience in home health care preferred.
Certificate/License:
Possession of a current license to practice asa physical therapist in the State of Connecticut.
General Summaryof Duties:
The provision of physical therapy to patients in theirhomes under written orders from qualified physicians according toestablished Agency policies and procedures.
Typical PhysicalDemands:
Requires full range of body motion. Requires ability toperform heavy work. Requires balance skills. Requires ability toascend/descend stairs. Requires ability to perform floor-to-standtransfers. Requires ability to kneel and sit for extended periods.
Requires ability to lift, transfer, and perform gait training onall surfaces with patients. Requires ability to lift and carryitems weighing up to 50 pounds. Requires corrected vision andhearing to normal range. Requires working under stressfulconditions. Requires limited exposure to communicable diseases.
Typical Work Conditions:
Work is performed in patients’ homes andin an office environment.
Knowledge, Skills and Requirements:
Knowledge of current APTA guidelines and standards. Knowledge ofAgency policy, regulations, and procedures to administer patientcare. Knowledge of current modalities and medical equipmentrelevant to patient care. Skill in identifying patient problems andrecommending solutions. Skill in documentation in clinical records.
Skill in establishing and maintaining effective workingrelationships with patients/families, physicians, co-workers, andthe community. Ability to communicate clearly. Possesses adequatetransportation to carry out job requirements. Must show proof ofsatisfactory automobile insurance with minimum coverage to bedetermined by Agency policy when using automobile fortransportation. Possession of an individual malpractice insurancepolicy. Willingness to provide service in any part of agencygeographic area, according to agency need. LPECHN
