General Summary of Duties:

The provision of physical therapy topatients in their homes under written orders from qualifiedphysicians according to established Agency policies and procedures.

Typical Physical Demands:

Requires full range of body motion.

Requires ability to perform heavy work•. Requires balance skills.

Requires ability to ascend/descend stairs. Requires ability toperform floor-to-stand transfers. Requires ability to kneel and sitfor extended periods. Requires ability to lift, transfer, andperform gait training on all surfaces with patients. Requiresability to lift and carry items weighing up to 50 pounds. Requirescorrected vision and hearing to normal range. Requires workingunder stressful conditions. Requires limited exposure tocommunicable diseases.

Typical Work Conditions:

Work is performedin patients’ homes and in an office environment. 1. C 4 , F 4Admits/readmits patients and provides physical therapy to patientsin their homes in compliance with Agency policies and State andFederal Regulations, incorporating the following: a) A completephysical assessment. b) Develops and implements an appropriate careplan and therapeutic goals with the patient/family. Arranges forprovision of assistive equipment. c) Evaluation of patient safetyon a 24-hour basis. d) Exercises appropriate clinical judgment incoordination of patient care by all services involved. e) Ongoingevaluation of significant changes in the condition of the patientand of progress toward the therapeutic goals. f) Use of Agency andcommunity consultants/resources. g) Instruction of patient/familyin procedures or responsibilities they are assuming and optimumwellness practices. h) Planning for the patient’s discharge fromservice. 2.

Establishes and maintains case mix weight,utilization, patient satisfaction that aligns with industrystandards and Agency operational objectives for quality care: a)OASIS documentation correlates with the Plan of Care. b) Usesrecommended OASIS assessment techniques 100% of the time. c) OASIS,documentation, and Plan of Care correlate to 95%. 3. C 4 , F 4Documents all services provided in the clinical record inaccordance with State and Federal Regulations and Agency policy: a)Documentation, including activities and mileage, if appropriate, isexported within 36 hours of visit. b) Obtains physicianverbal/written orders and complies with all reporting requirements.c) Participates in coordination of care with physicians and withother disciplines as appropriate. 4.

Manages workassignment in effective and efficient manner: a) Upon receipt ofreferral, makes phone contact with client and makes first homevisit according to Agency policy. b) Remains flexible in dailyplanning allowing time for emergencies or unexpected visits. c)Reports any cases to Clinical Manager where goals cannot be met orat risk situations exist and responds appropriately. d) Maintainsrequired productivity expectation as established by the Agency. e)Utilizes interdisciplinary approach, attending team meetings andinforming primary nurse of client’s progress/status with therapy.f) Delegates appropriate tasks to Home Care Aide. Orients andsupervises Home Care Aide in accordance with State and Federalregulations and Agency policy. g) Participates in the developmentof the interdisciplinary team Plan of Care on hospice patients,when appropriate. h) Utilizes payer sources correctly, reflectingchanging levels of care. i) Delegates patient care responsibilitiesto Physical Therapy Assistant as appropriate. j) Demonstrates anunderstanding of the principles of case management. k) Demonstratesability to manage cost and quality. 5.

Develops andmaintains sufficient professional skills and credentials to meetthe needs of patient care: a) Attends a minimum of 12 hours ofin-service per year to include mandated annual in-services. b)Participates actively in self-appraisal and the formation of a planof self-development. c) Adheres to Agency Corporate Compliance Planand HIPAA Privacy Plan as per written agreement. d) Seeksappropriate clinical input to ensure appropriate patient care. e)Projects professional image through attitude and compliance withdress code. f) Capable of precepting new Therapists and students.g) Functions as a team member with consideration to all staff. h)Provides Agency with current copy of professional license. i)Maintains CPR certification.

Knowledge, Skills and Requirements:

Knowledge of current APTA guidelines and standards. Knowledge ofAgency policy, regulations, and procedures to administer patientcare. Knowledge of current modalities and medical equipmentrelevant to patient care. Skill in identifying patient problems andrecommending solutions. Skill in documentation in clinical records.

Skill in establishing and maintaining effective workingrelationships with patients/families, physicians, co-workers, andthe community. Ability to communicate clearly. Possesses adequatetransportation to carry out job requirements. Must show proof ofsatisfactory automobile insurance with minimum coverage to bedetermined by Agency policy when using automobile fortransportation. Possession of an individual malpractice insurancepolicy. Willingness to provide service in any part of agencygeographic area, according to agency need.

Education:

Bachelor ofScience Degree in physical therapy from an APTA approved program.

Experience:

One year of acute care experience. Additionalexperience in home health care preferred.

Certificate/License:

Possession of a current license to practice as a physical therapistin the State of Connecticut.
