Position Summary:

TheOutpatient Physical Therapist administers Physical Therapy servicesunder medical prescription in accordance with Hospital regulation,State of Connecticut Practice Act and quality standards.

Responsible for clinical oversight of PTA Key duties and

Responsibilities:

Develops a plan of care for each patient with thepatient’s input and agreement, which includes safety, educational,psychological and developmental needs of the patient. Interpretsand assesses data about the patient as evidenced by appropriategoal setting and revision of goals throughout the plan care whileexplaining treatment goals, plans, techniques and outcomes topatient and or family. Displays expertise in recommending varioustherapeutic procedures and modalities. Complies with professionaland departmental documentation standards. Addresses a home exerciseprogram with each patient Maintains an adequate caseload, adheringto departmental productivity standards. Attends continuingeducation seminars as required by licensure and schedules andpresents an educational program, including tests, to Rehab staff orother medical professionals. Communicates regularly withRehabilitation Supervisor, other team members and referringphysicians for patient care. All other job related duties asassigned

Minimum Requirements:

Graduate of an accredited School ofPhysical Therapy Licensed or eligible for licensure to practice inthe

State of Connecticut Preferred Skills:

Experience withelectronic medical charting 3-5 years clinical experienceFlexibility with scheduling
