JobSummary and Qualifications Collaborates with colleagues in CareManagement department, service line, and interdisciplinary careteam to insure quality driven, best practice across the continuumof hospitalization by utilizing the eight essential activities ofcase management in direct patient/client contact: assessment,planning, implementation, coordination, monitoring, evaluation,outcomes and cost containment. Maximizes positive financialoutcomes for designated patient populations by intervening at keypoints in the disease process with post discharge follow-up phonecalls and crucial communication with care providers to promotehealth compliance and prevent readmissions. Provides designatedleadership functions for Care Management Department.
Whatqualifications you will need:
Required Experience:
RN with Bachelorof Science in Nursing or related field BSN or Bachelor degree inrelated field. Current licensure as Registered Nurse with the NorthCarolina Board of Nursing or a compact state as defined by NorthCarolina Board of Nursing. Current BCLS Maintains nationallyrecognized certification in Case Management (ACM or CCM, or ANAapproved certification) Minimum of Five years recent clinicalnursing experience in relevant practice area.
Preferred Experience:
Eight years recent clinical nursing experience in relevant practicearea. Five years managerial or Case Management experience.
