Job Summary andQualificationsThe Physical Therapist (PT) provides high-quality,patient-centered care by managing the complete process oftherapeutic care of an assigned group of patients in coordinationwith the physician and the patient care team. The PhysicalTherapist completes patient assessments and participates indeveloping and implementing individualized care plans focusing ongoal achievement, patient engagement, and positive patientexperience. The Physical Therapist provides or directs a majorityof the patient’s physical therapy to support patients throughoutthe continuum of care. What you will do in this role: You will provide evaluations and treatment interventions withinthe scope of practice established by law and professional code ofethics. You will assess and evaluatetherapeutic/rehabilitative/functional status and participate in thedevelopment of the total plan of care. You will provide purposeful and skilled therapeutic care atpatient’s home, using procedures and techniques to produce a changein condition/function consistent with the physician’s plan ofcare. You will instruct patient/family/caregiver in the use oftherapies, exercises, therapeutic appliances, and/or prostheticdevices to be used outside of therapy sessions. You will prioritize caseload, determine daily staffing needsand coordinate assignments/handoffs.

What qualifications you willneed: Licensed Physical Therapist must be obtained within 30 days ofemployment start date Drivers License Bachelors Degree is a healthcare organization serving westernNorth Carolina and offering a full continuum of post-acute care.
