POSITION SUMMARY
The Physical Therapist will primarilyprovide outpatient rehabilitation services in an in-home settingfor the older adult population. The in-home setting could be apatient’s private residence or in a patient’s room in anindependent living facility. Powerback Rehabilitation To You allowsthe Physical Therapist to evaluate and treat in the most functionalenvironment which is the patient’s home. The Physical Therapistassesses the need for, develops, and delivers physical therapyprograms to facilitate rehabilitation. The Physical Therapistidentifies the need for and makes referral(s) to other disciplinesto address the comprehensive needs of patients. They implementindividualized therapy programs designed to restore, reinforce, andenhance performance. In addition, the Physical Therapist instructs,educates and trains patients and caregivers in those skills andfunctions essential for promoting independence and productivity, inorder to diminish or correct pathology. They direct activities topromote and maintain health. This position provides a flexibleschedule and work environment in varioussettings.
RESPONSIBILITIES/ACCOUNTABILITIES
1. As necessary orapplicable, supervision of Physical Therapy Assistants, temporarylicensees, students and support personnel in accordance with statelicensure requirements and professional standards.2. Responsiblefor adherence to department and patient treatment schedule, timelyclinical and billing documentation requirements as well as workrelated meetings.3. Attends and contributes to patient care,staffing conferences and other related meetings.4. Makes referralsto other disciplines and agencies in order to address thecomprehensive needs of the patient.5. Consults with other membersof the rehabilitation team to select the most appropriatetherapeutic regimen consistent with the needs and capabilities ofeach patient.6. Adapts program and/or plan of care according to theneeds of the individual patient.7. Promotes a safe environment andenforces the elimination of fire and safety hazards.8. Orderssupplies and equipment as necessary.9. Cleans, maintains,identifies, and reports the need for repair of equipment asnecessary.10.

ensures that patients andfamilies receive the highest quality of service in a caring andcompassionate atmosphere which recognizes the individuals’ needsand rights.11.Screens, examines and evaluates patients, includinghistory, systems review, and application of appropriate tests andmeasures, synthesis information and interprets examination findingsin order to establish a diagnosis, identify impairments, determinethe predicted level of improvement and the time required to achieveit, identify precautions/contraindications and design and plan ofcare. Develops appropriate treatment goals and methods incollaboration with the patient and caregivers, implements thephysical therapy treatment plan, and completes all relateddocumentation and record keeping regarding these services. Servicesinclude but are not limited to application of therapeutictechniques in areas such as: Therapeutic exercise including aerobiccapacity conditioning, balance coordination, agility training, bodymechanics and postural stabilization, flexibility exercises, gaitand locomotion training, neuromotor development training,relaxation, strength, power, and endurance training Functionaltraining Manual therapy Airway clearance techniques Integumentaryrepair and protection Use of electrotherapeutic, physical agent andmechanical modalities Health and Wellness Education on ChronicDisease Management Falls Assessment and Interventions to reducefall risk Case Management in the home and community12. Designs,fabricates, fits and/or applies adaptive devices, and trainspatients in the use of adaptive devices included orthotics andprosthetics.13. Selects appropriate activities suited to theindividual’s physical and mental capacity, intelligence level andinterest.14. Instructs patient in the skills and techniquesrequired for participation in mobility activities and evaluatespatient progress, attitudes, and behavior as related torehabilitative potential.15. Performs routine reexamination asneeded/required to modify/progress plan of treatment.16.Incorporates health and wellness programming into treatment plansfor patients.17. Performs home and community mobility assessmentsand provides recommendations for safe mobility in the home andcommunity space.18. Performs other related duties asrequired.
SHAREPTHVRequirements:
1. Must have a valid PhysicalTherapy license in the state(s) of practice, or proof of licenseeligibility; and2. They must have a Master’s degree in PhysicalTherapy; or3. They must have a Master’s degree in a PhysicalTherapy related field in addition to a Bachelor’s degree inPhysical Therapy; or4. They must have a Bachelor’s degree inPhysical Therapy plus 5 years of progressively responsible PTexperience.5. The qualifying Physical Therapy degree earned outsideof the U.S. must be evaluated to be the U.S. equivalent to a degreein Physical Therapy.6. Must have a valid driver’s license orreliable, independent transportation to and from the patients’residence
