The ideal candidate will be responsible for evaluating, planning, and implementing rehabilitation programs to improve patients’ mobility and quality of life. This role is in a PPEC setting; experience in this setting is preferred. Duties Conduct thorough evaluations of patients to assess their physical abilities and limitations. Develop and implement personalized treatment plans based on patient assessments and goals. Provide hands-on therapy techniques, including exercises and modalities, to enhance recovery. Collaborate with other healthcare professionals in discharge planning and ongoing patient care. Educate patients and their families about treatment processes, exercises, and home care strategies. Maintain accurate documentation of patient progress and treatment outcomes. Stay updated on the latest advancements in physical therapy practices and medical terminology. Work effectively with diverse populations including pediatrics, geriatrics, and orthopedic patients. Requirements A degree in Physical Therapy from an accredited program. Valid state licensure or eligibility for licensure as a Physical Therapist. Strong knowledge of medical terminology related to physical therapy practices. Experience in acute care settings is preferred but not mandatory; familiarity with home community care is a plus. Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams. Ability to develop discharge plans that ensure continuity of care post-treatment. A commitment to providing high-quality patient care across various demographics including pediatrics and geriatrics. Familiarity with physical therapy systems and protocols.
