Job Function:

Providesand directs Physical Therapy services to residents, includingassessment, treatment, program planning and implementation, relateddocumentation, and communication. Functions under physicians’orders.

Responsibilities Include:

• Provides Physical Therapyservices to residents and initiates referrals when appropriate andin accordance with department procedures.
• Conduct screenings ofresidents at regular intervals to determine the need forintervention/treatment.
• Evaluates residents to obtain datanecessary for treatment planning and implementation.
• Conductspecialized evaluations as indicated.
• Adheres to establishedconfidentiality standards.
• Implements and conducts treatment asoutlined in treatment plan.
• Follows management direction.
• Performs other duties as assigned.

Education and Experience:

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination forRegistered Physical Therapist.
• Current state licensure or licenseeligible.
