Job Summary and Qualifications Physical Therapist -Full-time, inpatient rehab The Physical Therapist (PT) provideshigh-quality, patient-centered care by managing the completeprocess of therapeutic care of an assigned group of patients incoordination with the physician and the patient care team. ThePhysical Therapist completes patient assessments and participatesin developing and implementing individualized care plans focusingon goal achievement, patient engagement, and positive patientexperience. The Physical Therapist provides or directs a majorityof the patient’s physical therapy to support patients throughoutthe continuum of care. What you will do in this role: You will provide evaluations and treatment interventions withinthe scope of practice established by law and professional code ofethics. You will assess and evaluatetherapeutic/rehabilitative/functional status and participate in thedevelopment of the total plan of care. You will provide purposeful and skilled therapeutic care atpatient’s home, using procedures and techniques to produce a changein condition/function consistent with the physician’s plan ofcare. You will instruct patient/family/caregiver in the use oftherapies, exercises, therapeutic appliances, and/or prostheticdevices to be used outside of therapy sessions. You will prioritize caseload, determine daily staffing needsand coordinate assignments/handoffs.
