• We are seeking a dedicated and compassionatePhysical Therapist to join our team. The ideal candidate will havea strong background in home health care. This role involves workingwith diverse patient populations, including geriatrics, orthopedicsand neuro, to develop and implement effective treatment plans thatenhance mobility, relieve pain, and improve overall physicalfunction.
• Responsibilities
• Conduct thorough evaluations ofpatients’ physical abilities and limitations.
• Developindividualized treatment plans based on patient assessments andgoals.
• Implement therapeutic exercises and interventions toimprove strength, flexibility, and coordination.
• Educate patientsand their families on treatment protocols and home carestrategies.
• Collaborate with other healthcare professionals toensure comprehensive patient care.
• Maintain accurate documentationof patient progress and treatment outcomes.
• Stay updated on thelatest advancements in physical therapy practices andtechniques.
• Requirements
• Valid state license to practice as aPhysical Therapist.
• Strong knowledge of anatomy and physiologyrelevant to physical therapy.
• Experience in geriatrics, orthopedicand home community care is preferred.
• Familiarity with kinnsersystem for patient management and documentation.
• Excellentcommunication skills for effective interaction with patients andhealthcare teams.
• Ability to work independently as well as part ofa collaborative team environment.
• Commitment to ongoingprofessional development and continuing education in the field.
