Job Function:

Provides and directs Physical Therapy services toresidents, including assessment, treatment, program planning andimplementation, related documentation, and communication. Functionsunder physicians’ orders.

Responsibilities Include:

• Provides Physical Therapy services to residents and initiatesreferrals when appropriate and in accordance with departmentprocedures.
• Conduct screenings of residents at regular intervals todetermine the need for intervention/treatment.
• Evaluates residents to obtain data necessary for treatmentplanning and implementation.
• Conduct specialized evaluations as indicated.
• Adheres to established confidentiality standards.
• Implements and conducts treatment as outlined in treatmentplan.
• Follows management direction.
• Performs other duties as assigned.

Education and Experience:

• Bachelor of Science in Physical Therapy from an accreditedprogram.
• Successful completion of National Certification Examination forRegistered Physical Therapist.
• Current state licensure or license eligible.
