Position Overview Responsible for providing medically prescribed physical therapy to patients in the home in accordance with agency policies. Participates in developing and implementing the plan of care with goals and documentation appropriate to discipline guidelines.

Provides supervision as needed to PTA per JCAHO/CHAP, Florida State and Federal practice act guidelines .Performs physical therapy procedures, evaluations, resumption of care, admissions, and diagnostic tests on patients for whom these have been medically prescribed and establishes measurable goals. Teaches appropriate treatment procedures and supportive activities to patients and family members. Minimum Qualifications BS degree and graduation from an accredited school of Physical Therapy required. Work experience of at least one year as a Physical Therapist, Home Health experience preferred. Able to prioritize and manage time effectively and make independent decisions when necessary. Current Florida Physical Therapist License. CPR certification. Valid Florida or Alabama Driver’s License and current
