We are looking for a PRN Physical Therapist to pick up cases in the Orange County area. This is a growing private duty Skilled Home Health Care agency and is Non-Medicare/Non Oasis. Great for someone with Flexibility in their schedule to make their own hours. We are seeking a dedicated and compassionate Physical Therapist to join our team in the home health environment. You will play a crucial role in helping patients regain mobility and improve their quality of life through tailored therapeutic interventions. Duties Assist in implementing individualized treatment plans. Conduct patient assessments and monitor progress towards rehabilitation goals. Provide hands-on therapeutic interventions, including exercises and modalities, to enhance patient recovery. Educate patients and their families on home exercise programs and self-care strategies. Participate in discharge planning by collaborating with the healthcare team to develop comprehensive care plans. Maintain accurate documentation of patient care activities and progress notes in compliance with medical standards. Uphold knowledge of medical terminology and apply it effectively in patient interactions and documentation. Work with diverse populations ensuring sensitivity to their unique needs.

Experience Ideal candidates will possess:

A strong understanding of physiology and its application in physical therapy practices. Experience in home health settings or outpatient therapy environments is preferred. Familiarity with discharge planning processes and the ability to contribute to effective care plans.
